IMPORTANT: Applicant's parent or legal guardian must sign and return the Parental Liability Waiver and the Medical Authorization Form prior to the player attending Malibu Baseball Camp.

The forms may be filled out online to print, downloaded below or sent to you by the US Postal Service so you may fill them in by hand, or you may fill them in online during your online application process. If you choose to fill them in online, you will be presented with the opportunity to print the filled in forms so you may sign and return them to the camp by mail, or hand carry them to the FIRST day of camp.

The forms must have the signature of a parent or legal guardian.
